Video 3.19 covers the entire dorsal roof technique from beginning to end. The dorsal roof, the strip of cartilage and bone that carries the dorsal aesthetic lines, is separated from the septum and the lateral walls, the septum is lowered, and the roof is fixed back like spreader flaps at the desired height; the upper lateral cartilages are stabilised as spreader flaps and the bony roof is then narrowed with osteotomies.

The technique was developed to overcome the limitations of the down techniques while still preserving the natural dorsum, and the figures accompanying the video show three- and five-year results with a narrower, straight and anatomical dorsum. Video 3.20 explains the same principles in an animation.
