Deep Plane Facelift
In a deep plane facelift the SMAS layer and the skin above it are released and repositioned as a single unit, instead of the skin being pulled over a separately tightened SMAS. Releasing the retaining ligaments of the midface lets the cheek and the jawline move along their natural vector, which is why a well-executed deep plane result tends to look rested rather than pulled. The dissection runs in defined planes in relation to the facial nerve branches, so it is a technically demanding operation that depends on precise anatomical knowledge and experience.
Scarless and Endoscopic Facelift
In selected patients — usually faces where skin quality is still good and the main problem is descent rather than excess skin — the upper and middle thirds of the face can be lifted endoscopically through short incisions hidden inside the hairline, combined with temple suspension. This is what is usually meant by a “scarless” facelift: the incisions are short and concealed, not absent. It is not a substitute for a full facelift when there is significant laxity in the neck.
Mini, Full and Revision Facelift
A mini facelift addresses limited jawline laxity through a shorter incision and a smaller dissection. A full facelift treats the midface, the jawline and the neck together and usually includes a neck lift. A revision facelift is performed after a previous lift, where scar tissue, an altered blood supply, a displaced hairline or an over-tightened result has to be corrected; it is planned differently from a first operation and often needs fat grafting to restore volume rather than more tension.
Recovery After a Facelift
Most patients are up and walking the day after surgery. Bruising and swelling are most noticeable during the first week, and sutures in front of the ear are usually removed between the fifth and the seventh day. Social recovery — being comfortable in a meeting or a restaurant — is generally two to three weeks, while the final contour keeps settling over several months as the tissues soften.

What is a deep plane facelift and how does it differ from a SMAS facelift?
In a deep plane facelift the SMAS and the skin are lifted together as one unit after the midface retaining ligaments are released, so the tissues move along their natural vector. In a classic SMAS facelift the skin is lifted separately over a tightened SMAS, which puts more tension on the skin and can look tighter around the mouth and ear.
Is a scarless facelift really without scars?
No. A scarless facelift means the incisions are short and hidden inside the hairline or in natural creases rather than absent. The term describes how well the scars are concealed, not the absence of an incision.
Who is a candidate for an endoscopic facelift?
Endoscopic lifting suits patients whose main problem is descent of the brow, temple and midface while the skin still has good elasticity, and where there is little excess skin in the neck. When the neck is significantly lax, an endoscopic lift alone will not be enough.
When can a revision facelift be performed?
A revision is usually planned at least twelve months after the previous operation, once swelling has settled and the scar tissue has matured.
