Case Study 5: Facelift, Neck Lift, Temporal Lift, Fat Grafting and Eyelid Surgery at 48
Updated: Sep 18
Case summary
Patient: 48-year-old woman from Turkey
Procedures: facelift, neck lift, temporal lift, fat injection, upper blepharoplasty, laser skin tightening
Surgeon: Prof. Dr. Suleyman Tas, Prof. Dr. Suleyman Tas Clinic, Istanbul
Goal: a naturally younger face and neck, with volume restored under the eyes

The patient's concerns
In her late forties this patient noticed that her face and neck had begun to sag and that hollows had formed beneath her eyes, making her look tired even when rested. Her priorities were a firmer face and neck and a fresher look around the eyes, without the stretched appearance she associated with older facelift techniques.
Why lift and volume together
Facial ageing has two components. Tissues descend, which is corrected by lifting, and tissues lose volume, which is corrected by adding volume. A facelift alone can make a hollow face look tight and thin; fillers alone can make a sagging face look heavy. Treating both gives the most natural rejuvenation, which is why this patient's plan combined surgical lifting with fat grafting.
What was done
Facelift and neck lift
The deep layers of the face and neck were repositioned and the muscle of the neck was tightened, redefining the jawline and removing the loose skin and bands of the neck. Because the lift is performed in the deeper layer, the skin is not pulled tight and the result looks natural. Learn more on the facelift page.
Temporal lift
A temporal lift elevated the outer part of the eyebrows and the surrounding temple skin, lifting the corner of the eyes and softening crow's feet.
Upper blepharoplasty
Excess skin, and where needed fat and a small amount of muscle, was removed from the upper eyelids, opening the eyes. See blepharoplasty in Turkey.
Fat injection
The patient's own fat, harvested gently by liposuction, was purified and injected into the under-eye hollows and other areas of volume loss. Fat grafting restores youthful fullness and can improve skin quality.
Laser skin tightening
Laser energy was used to stimulate new collagen, improving skin firmness and texture as a finishing touch to the surgical lift.
Result
The combined procedures gave a firmer jawline and neck, lifted brows, rested eyes and softened under-eye hollows. The overall effect is a naturally refreshed version of the patient, consistent with Prof. Dr. Tas's philosophy of rejuvenation without an operated look.
Frequently asked questions
Is fat grafting permanent?
A proportion of transferred fat is lost in the first months; what survives after about three months is generally long-lasting.
How long is recovery after facelift and neck lift?
Most patients are socially comfortable at two to three weeks, with final results at three to six months.
Can the face look pulled after a facelift?
Not when the deeper layers are lifted rather than the skin. Natural results are the goal.

How long do facial rejuvenation results last?
A surgical lift typically holds for around a decade because it is carried by the deeper layer rather than the skin. Ageing continues, so sun protection, skin care and non-surgical maintenance still matter.
Where are the scars?
In the natural creases in front of and behind the ear and inside the hairline, placed so that hair and make-up cover them while they fade over the following months.
