Rhinoplasty 7 Days After Surgery: What Your Nose Looks Like When the Cast Comes Off
Quick answer: Seven days after rhinoplasty, the cast or splint is removed and you see the new shape of your nose for the first time. At this point the nose is still swollen, especially at the tip, the tip may look slightly upturned and the skin may feel numb. Bruising is usually fading or already gone. Most visible swelling settles within 2 to 3 weeks, while the tip keeps settling for 12 months or longer in thick skin.
Key takeaways
The cast usually comes off around day 7; you see the new profile immediately.
The nose looks swollen and slightly upturned at first; this is normal.
With the closed atraumatic technique, bruising is often mild or absent by day 7.
Most people return to work and social life after 7 to 10 days.
The final shape, especially of the tip, appears over 6 to 12 months.
What happens on day 7?
The cast protects the new shape of the bones and cartilage during the first week. On day 7 it is removed together with the tape and any internal silicone splints. Patients are often surprised that this is painless. Then comes the moment many have waited months for: the first look in the mirror.

What is normal at one week?
Swelling: the bridge may look wider and the tip fuller than the final result.
Upturned tip: the tip often looks slightly higher at first and settles over weeks.
Numbness: the tip and upper lip can feel numb or stiff; sensation returns over months.
Mild bruising: yellowish shadows under the eyes may remain and can be covered with make-up.
Blocked nose: breathing through the nose can feel congested for 2 to 3 weeks.
Why is bruising often lower with the closed atraumatic technique?
In the closed atraumatic rhinoplasty technique developed by Prof. Dr. Suleyman Tas and published in Plastic and Reconstructive Surgery in 2014, all incisions are inside the nose and the soft tissues and ligaments are preserved as much as possible. Less tissue disruption usually means less swelling and bruising in the first week. Many patients are able to go out without visible bruises at day 7, as in the videos on this page.

Some patients use the TAS Nose Cast, a cast designed so it can be removed easily. Learn more on the TAS Nose Cast page.
"The first week shows the direction of the result, not the result itself. I always tell my patients: judge the profile at one week, judge the tip at one year." — Prof. Dr. Suleyman Tas
What changes after the first week?
Weeks 2 to 3: most visible swelling settles; the nose looks natural in photographs.
Months 1 to 3: the bridge and profile become sharper.
Months 6 to 12: the tip becomes more defined; in thick-skinned noses this can take longer.

Do's and don'ts after the cast comes off
Do clean the nostrils gently as instructed and use the saline spray you were given.
Do sleep with your head slightly raised for another week.
Do not blow your nose hard for about 2 weeks.
Do not wear heavy glasses on the bridge for about 4 to 6 weeks.
Avoid intense exercise for about 4 to 6 weeks, and contact sports where the nose could be hit for about 3 months.
Bruising: what helps
Keep your head raised with two pillows for the first week.
Use cold compresses around the eyes during the first 48 hours.
Avoid aspirin, ibuprofen and supplements that thin the blood unless your doctor allows them.
Avoid hot baths, saunas and strong sun for a few weeks.
When to contact the clinic
Contact the team if you notice increasing pain, fever, bleeding that does not stop with gentle pressure, or one side becoming much more swollen than the other. These are uncommon, but early review keeps small problems small.
Thick skin and the first week
Patients with thick skin usually look more swollen at day 7, especially at the tip. This is expected and does not predict the final result. Taping and, in some cases, small injections in later months help the tip settle.
Frequently Asked Questions
Does cast removal hurt?
No. The cast and tape are lifted off gently. Removing internal silicone splints can feel strange for a moment, but it is not painful.
Why does my nose look bigger after the cast comes off?
Because of swelling. The nose is usually at its most swollen in the first days after cast removal and slims down over the following weeks.
Can I fly home 7 days after rhinoplasty?
Most international patients fly home after the cast is removed and the nose is checked, usually on day 7 or 8.
When can I go back to work?
Most people return to office work 7 to 10 days after surgery.
When will I see the final result?
The profile is clear within weeks, but the final shape of the tip appears over 6 to 12 months.
