Should You Dissolve Fillers Before Facelift Surgery?

Quick answer. Not always. Hyaluronic acid (HA) filler can be dissolved with an enzyme called hyaluronidase, and a 2025 surgeon panel recommends giving it at least two weeks before facial surgery when old HA filler sits in areas that worry the surgeon, such as under the eyes. Other filler types cannot be dissolved. Whether you need it depends on how much filler you have and where it sits.

Key takeaways

  • Prior injectables do not automatically rule out a facelift.

  • Only hyaluronic acid filler responds to hyaluronidase; calcium hydroxylapatite, poly-L-lactic acid and permanent fillers do not.

  • A 2025 surgeon panel advises hyaluronidase at least two weeks before surgery when it is needed.

  • HA filler can stay in tissue much longer than its label suggests.

  • Bring a full list of your treatments, dates and any complications to your consultation.

Why do fillers matter to a facelift surgeon?

A facelift works by moving tissue layers that normally glide over each other. Filler sits inside those layers. Usually that is not a problem. But when filler has been placed in large amounts, in the wrong plane, or has caused inflammation, it can blur the layers the surgeon needs to separate.

A 2025 consensus paper by seven plastic surgeons and one dermatologist explained that the main concern is not filler itself but distorted tissue planes and reduced blood supply (Shridharani et al., 2025). The same panel rated HA fillers as the injectables least likely to cause problems at surgery, with permanent fillers posing higher risks.

Filler can also hide what your face is really doing. Volume in the cheeks or jawline may mask how far the tissue has descended. Once it is gone, the true degree of sagging becomes clear, and the surgical plan may change.

How long does filler stay in the face?

Longer than many patients expect. A 2026 review reported that the visible effect of HA gels generally lasts 4 to 18 months (Alizadeh et al., 2026). The material itself can persist far longer. One case series found HA filler in tissue up to 9 years after injection (Chang et al., 2017), and the 2025 panel noted that HA fillers often persist longer than product labels state (Shridharani et al., 2025).

So "I last had filler three years ago" does not mean it has all gone.

Do previous fillers increase facelift complications?

The evidence so far is reassuring but limited. In a 2025 review of 212 deep plane facelift patients, 57% had prior injectables (Salsberg et al., 2025). Complications occurred in 16% of those with prior injectables and 8% of those without, a difference that was not statistically significant. A smaller 2026 chart review of 20 patients found no significant complications caused by prior filler (Myers et al., 2026).

Neither study is large enough to prove there is no added risk. They do show that people with filler histories routinely have facelifts.

What is hyaluronidase and what can it dissolve?

Hyaluronidase is an enzyme that breaks down hyaluronic acid. It works only on HA filler. It does not dissolve:

  • calcium hydroxylapatite

  • poly-L-lactic acid

  • permanent fillers such as PMMA or silicone

In the European Union and Canada, using hyaluronidase to dissolve cosmetic filler is off-label (Arrigoni et al., 2025). Allergic reactions are rare, estimated at 0.05% to 0.7%. People allergic to bee or wasp stings are at higher risk because insect venom contains hyaluronidase, so tell your doctor about any such allergy (Arrigoni et al., 2025).

How long before a facelift should fillers be dissolved?

The 2025 panel recommends hyaluronidase at least two weeks before surgery, because the enzyme can keep breaking down HA gradually for up to two weeks (Shridharani et al., 2025). The panel also advised against facial surgery within two weeks of any treatment that causes inflammation.

  • Hyaluronidase to dissolve HA filler — Suggested wait before facial surgery (2025 panel): At least 2 weeks

  • Deoxycholic acid (fat-dissolving) injections — Suggested wait before facial surgery (2025 panel): At least 1 month, ideally 6 months

  • Absorbable threads — Suggested wait before facial surgery (2025 panel): 6 to 9 months

  • Microneedling without radiofrequency — Suggested wait before facial surgery (2025 panel): At least 6 months

  • Subdermal radiofrequency treatments — Suggested wait before facial surgery (2025 panel): Collagen remodelling may continue for up to 12 months

These are panel opinions, not trial results. Your surgeon may adjust them after examining you.

Should you dissolve everything if your face looks overfilled?

Not necessarily. Some patients have a heavy, puffy look from layered filler, often called overfill. A 2026 review argued that overfill is not caused by the product itself but by technique, placement or patient selection (Alizadeh et al., 2026).

Dissolving can be targeted rather than total. Under-eye filler is a common target because it can cause puffiness and a bluish tint. In one study of 23 patients treated before lower eyelid surgery, 15 needed one hyaluronidase session and 8 needed two; in 4 patients, mild swelling persisted even when the filler had gone (Zoumalan, 2018). So expect improvement, not always a complete reset.

Can filler be managed during the facelift itself?

Sometimes. The 2025 panel described surgeons using hyaluronidase during the operation to ease dissection and to deal with HA filler or nodules found in the tissue (Shridharani et al., 2025). Ask your surgeon whether they plan to dissolve filler before surgery, manage it during surgery, or leave it alone, and why.

How Prof. Dr. Suleyman Tas approaches this

Tell Prof. Dr. Suleyman Tas about every filler, thread and device treatment you have had, because it shapes the plan. He performs deep plane face and neck lifts under local anesthesia, with you awake, one patient per day, personally. Because his TAS Signature Lift can include fat injection alongside a deep plane facelift, neck lift and eyelid surgery, volume can be planned as part of the same session where your face needs it. Read about facial fat transfer and learn when fillers stop working.

You stay in Istanbul for about 10 to 14 days, with daily hyperbaric oxygen sessions and a private nurse, then one year of online follow-up. The fee is quoted individually after Prof. Dr. Tas has reviewed your photographs or examined you. See how to prepare for a deep plane facelift.

Frequently Asked Questions

Do I need to stop fillers before a facelift?

It is sensible to stop new filler injections well before surgery so the tissue is settled and not inflamed. A 2025 surgeon panel advised that injectables should be given sufficiently in advance of surgery. Ask your surgeon for a specific date, and do not book new filler once your facelift is planned.

Will hyaluronidase dissolve my natural tissue too?

Hyaluronidase breaks down hyaluronic acid, which your own tissues also contain. The 2025 surgeon panel describes its effect on the surrounding tissue matrix as temporary. Dosing and protocols vary widely between practitioners, according to a 2025 review. Discuss the dose, the exact areas to be treated and what to expect afterwards before any injection.

Can I have a facelift if I had collagen-stimulating fillers?

Usually, yes. Poly-L-lactic acid and calcium hydroxylapatite are not dissolved by hyaluronidase. A 2025 surgeon panel rated them as somewhat higher risk than HA filler, but lower risk than permanent fillers, for later surgery. Your surgeon plans around them. Tell your surgeon exactly which product, where it was injected and when, ideally with records from your injector.

Can I have filler after my facelift?

Many patients do. In one small 2026 chart review of 20 facelift patients, 60% had filler within the first year after surgery. Wait until your surgeon confirms healing is complete, and tell your injector you have had a deep plane facelift so they understand the new anatomy.

What information should I bring to my consultation?

Bring the names of products used, dates, areas, the number of syringes if known, any hyaluronidase treatments, and any complications such as lumps, infection or skin colour change after injection. Photos from before your first filler help too, because they show how your face looked before volume was added.
