Methods and Clinical Approach

The pivot point is planned 4-5 mm more cephalically than the dome, and the medial rather than lateral flap surface is sutured to the septum. The modification is designed to preserve a supratip break and improve coaptation.

Results

The anatomic rationale indicates that the modified flap can combine spreader-graft behavior, lateral-crural suturing, supratip-break creation and nasolabial-angle support.

Clinical Significance

The letter presents a targeted technical refinement for selected rhinoplasty patients and links flap placement directly to tip projection, rotation and contour.
