Management of Proximal Third Arm Replantation

Methods and Clinical Approach

A 47-year-old man sustained crush-avulsion amputation through the proximal arm. Bone shortening and fixation were followed by brachial-artery, three-vein and major-nerve repair. Delayed fasciotomy and negative-pressure therapy controlled severe edema before skin grafting.

Results

At one year, bone union was stable, the patient had shoulder motion, limited elbow flexion, protective hand sensation and a Chen grade 3 helper-arm result.

Clinical Significance

Careful patient selection, rapid revascularization, multidisciplinary support, compartment surveillance, tailored fasciotomy and long rehabilitation are essential in proximal arm replantation.
