Spontaneous Rupture of the Extensor Pollicis Longus Tendon due to Unusual Etiology

Methods and Clinical Approach

A 40-year-old woman without fracture, rheumatoid arthritis, steroid exposure or other usual risk factors presented with wrist pain and inability to extend the thumb. Surgical exploration found a degenerated distal EPL stump.

Results

The extensor indicis proprius tendon was transferred to the EPL using the Pulvertaft technique. At one year the patient regained active thumb extension.

Clinical Significance

The case highlights repetitive wrist and thumb motion as a possible mechanical contributor when common systemic and traumatic causes are absent.
