Methods and Clinical Approach

A 50-year-old patient with congenital vaginal agenesis underwent transabdominal guidance, creation of a new vaginal pouch, and reconstruction with a full-thickness skin graft wrapped around an inflatable mold.

Results

Full graft take was seen at day seven. At one year the neovagina measured 12 x 4 cm; no hemorrhage, infection, fistula or hypertrophic donor-site scar was reported, and the patient described satisfactory function.

Clinical Significance

Regular postoperative mold use remains central to preventing contracture. Transabdominal guidance can improve anatomic orientation and safety in complex secondary cases.
