I thank Dong and Fan for their commentary and will enlarge the scope of my article, “Ultra Diced Cartilage Graft in Rhinoplasty: A Fine Tool,”1 with further discussion and comment for the readers. The size of diced cartilage pieces. The study exposed the dead space that occurs between large pieces of cartilage, which is one of the most important causes of graft resorption. Since lack of contact with viable surfaces causes this complication, it can be eliminated with the ultra-diced cartilage grafting technique. Thus, dicing the cartilage as small as possible is logical to increase the viability index and decrease the visibility problem. Evaluation of absorption and follow-up. The study was very unique and original and one of the strongest clinical studies in the literature of aesthetic surgery, due to it being a level 3 study and its methodology, which compared preoperative standardized photographs with 1- year postoperative images, generally accepted as the final result in plastic surgery. Any type of change, such as absorption or displacement, can be effectively judged before swelling by recording the status before and 1 year after surgery. Preoperative three-dimensional digital scanning, ultrasound, and computed tomography not only would be poorer choices than standardized photography for evaluation but also would not be practical nor ethical. Preparation time for ultra-diced cartilage. The procedure takes less than 2 minutes for 1 cc of ultra-diced cartilage. [See Video 1 from the original article,1 which shows all of the steps of the ultra-diced cartilage graft technique.] The presented ultra-diced cartilage method is one of the most important innovations in rhinoplasty because it provides perfect cartilage material in just 2 minutes using its amazing, simple, and practical dicing method. The design of two insulin injectors with 15 blades is an original solution that has not been seen before.1 This was a preliminary study of my ultra-diced cartilage grafting technique. Further details and studies are shared in my upcoming book.2 In addition, many practical surgical maneuvers, including the ultra-diced cartilage grafting technique, are shared in my course,3 which is organized annually.
