Wide Nose Rhinoplasty: How a Wide Nose Is Narrowed Without Harming Breathing

4 min read

Quick answer: Wide nose rhinoplasty narrows the parts of the nose that make it look wide: the bony bridge, the cartilage of the tip and, when needed, the nostrils. The bridge is narrowed by repositioning the nasal bones, the tip by reshaping cartilage and the nostrils by small incisions hidden in the natural crease. The nose must be narrowed without collapsing the inner airway, so breathing is protected at every step.

Key takeaways

  • A nose can look wide because of the bones, the tip, the nostrils or the skin.

  • Each part is treated with a different technique, so a precise analysis comes first.

  • Narrowing must protect the internal nasal valve, the narrowest point of the airway.

  • Thick skin limits how narrow the tip can look and needs a specific approach.

  • The front-view change is often visible at day 7 and keeps improving for months.

Why does a nose look wide?

A front-view analysis usually shows one or more of four causes:

  • Wide bony bridge: the nasal bones sit far apart.

  • Wide or bulbous tip: the tip cartilages are broad or rounded.

  • Wide nostrils (alar base): the nostrils flare beyond the inner corners of the eyes.

  • Thick skin: heavy skin hides the shape of the cartilage underneath.

The video above, with more than 4.6 million views, shows how much a wide nose can be narrowed on the operating table and the narrower, natural-looking nose right after surgery.

How each part is narrowed

  1. Bridge: controlled cuts in the nasal bones (osteotomies) allow them to be moved closer together.

  2. Tip: the tip cartilages are reshaped with sutures, and excess fatty tissue under thick skin can be carefully thinned.

  3. Nostrils: a small wedge of skin is removed in the natural crease where the nostril meets the cheek; the scar is usually very hard to see.

  4. Support: when needed, grafts from the patient's own septum cartilage support the airway so breathing is not narrowed.

Why breathing comes first

The internal nasal valve, the area just under the middle of the bridge, is the narrowest part of the airway. If a surgeon narrows the bridge without supporting this area, the nose may look slimmer but breathe worse. In Prof. Dr. Suleyman Tas's closed atraumatic technique, the supporting ligaments and structures are preserved and, when needed, reinforced. This is part of his approach described in his textbook Rhinoplasty in Practice (CRC Press, 2020).

"A narrow nose that does not breathe is not a success. Every millimetre we take from the width must be matched with support inside." — Prof. Dr. Suleyman Tas

Thick skin and ethnic noses

Many wide noses have thick skin, for example in Middle Eastern, African or Asian patients. Thick skin does not shrink much, so making the tip very small can create a shapeless, swollen tip. The goal is a nose that keeps the patient's identity, with better definition and breathing. Read more in thick-skinned nose rhinoplasty and ethnic rhinoplasty.

Recovery

  • Day 7: the cast is removed; the front view is already narrower.

  • Weeks 2 to 3: most swelling settles; nostril stitches are out and the scars keep fading over the following months.

  • Months 6 to 12: the tip becomes more defined, more slowly in thick skin.

Who is a good candidate?

You may benefit from wide nose rhinoplasty if your nose looks wide in photographs, if the bridge appears flat and broad, if the tip is round or heavy, or if the nostrils flare when you smile. Good candidates are healthy, do not smoke and want a natural improvement rather than a very small or pinched nose.

Risks

Possible risks include prolonged swelling, minor asymmetry, temporary numbness of the tip, visible nostril scars in rare cases and breathing problems if the airway is not supported. Choosing a surgeon who plans both shape and function is the most effective way to reduce them.

How to prepare for your consultation

Send clear photographs from the front, both sides, three-quarter views and from below, in daylight and without filters. Mention any breathing problems, previous nose injuries or earlier nose operations, because they change the plan.

Frequently Asked Questions

Can a wide nose be narrowed without visible scars?

The bridge and tip are narrowed from inside the nose. Only nostril reduction needs small incisions in the natural crease beside the nostrils, which usually heal very discreetly.

Will narrowing my nose affect my breathing?

It should not, when the airway is supported during surgery. A proper technique aims to keep or improve breathing.

Is nostril reduction always necessary?

No. Many wide noses are narrowed at the bridge and tip only. Nostril reduction is added only when the nostrils themselves are wide.

How long until I see the result?

The front view is narrower at day 7, but the final definition, especially in thick skin, appears over 6 to 12 months.

Can a wide nose rhinoplasty keep my ethnic features?

Yes. The aim is a narrower, better defined nose that still suits your face and background.
