Video 4.20: Untreated Blinking Nose Deformity Causing a Supratip Deformity Two Years After Rhinoplasty

What this video shows

Video 4.20 shows why the blinking nose deformity must be recognised before rhinoplasty. A 28-year-old male patient had been operated two years earlier, and a supratip deformity was visible despite skin care and taping in the postoperative period. The powerful contraction of the muscle is remarkable in the video: it does not allow the dissected supratip skin to settle on the groove, the skin shortens itself and the supratip bulges.

The book states plainly that if a blinking nose deformity is not treated, a supratip deformity will appear despite all measures taken; this case is the clinical proof and the reason the condition is examined in every patient.

Key points

  • Supratip deformity two years after rhinoplasty in an untreated blinking nose.

  • Muscle contraction prevents the supratip skin from settling.

  • Illustrates the importance of preoperative diagnosis (Video 4.16).

For patients: a bump above the tip after surgery is not always the surgeon's technique; sometimes a small, overlooked muscle is responsible, and it can be treated.
