Rhinoplasty in Practice: An Algorithmic Approach to Modern Surgical Techniques is the rhinoplasty book written by Prof. Dr. Suleyman Tas and published by CRC Press, an imprint of Taylor & Francis (first edition 2021, © 2022). It is a single-author surgical textbook that turns closed atraumatic, dorsum-preservation, structural and revision rhinoplasty into step-by-step algorithms, and it comes with a library of full-HD surgical videos. Fifty of those companion videos are published on this page, each with its own written explanation.

About the book

The book grew out of the author's own difficulty in finding a clear, algorithmic source when he moved from super-microsurgery and facial reconstruction to rhinoplasty. With more than 11,000 papers and 100 books on the subject, the problem for a surgeon is no longer a lack of information but the lack of a holistic, decision-based approach. The seven chapters follow the order of a real operation and of the author's own practice: patient selection, the closed atraumatic technique, dorsum surgery, nasal tip surgery, alar base surgery, revision rhinoplasty and advanced rhinoplasty.

Every technique is presented with its anatomy, indications, limitations and complications, with algorithms and tables that show which technique to choose for which deformity, and with tips drawn from thousands of operations. The illustrations include cadaver, histological, ultrasonography and CT studies, and the surgical steps are demonstrated in the companion videos.

The seven chapters

• Chapter 1 – Patient Selection in Rhinoplasty: patient identification, physical, psychological, functional and aesthetic evaluation, simulation study and the revisit; the psychological evaluation form used in the author's practice is included as an appendix.

• Chapter 2 – Closed Atraumatic Rhinoplasty: why the philosophy begins with the closed technique; local injection, incisions, the submembranous dissection plane and the “room concept”, preservation, reshaping, reconstruction, structure and redraping; open–closed and submembranous–subSMAS comparisons.

• Chapter 3 – Dorsum Surgery: osteotomy and its complications, osteoplasty and maxilloplasty; composite and component hump removal, dorsum reconstruction and the bone dust technique; dorsum preservation with the down techniques and the dorsal roof technique; the dorsum algorithm in reduction rhinoplasty; dorsum augmentation with the ultradiced cartilage method.

• Chapter 4 – Nasal Tip Surgery: the tripod concept, supratip break point, nostril image, lateral and medial crus malposition and the bifid nose; lower lateral cartilage flaps, the lateral crural flap, the ST flap (superior-based transposition flap) and the reverse ST flap; the anatomic strut and the columellar strut algorithm; the anatomic columellar septal extension graft; the Pitanguy ligament, deep SMAS suture, perichondrio-SMASectomy and soft-tissue redraping; electrocautery; the blinking nose deformity and its treatment.

• Chapter 5 – Alar Base Surgery: the alar excision algorithm, the alar base narrowing suture, symmetry in the vertical, horizontal and sagittal planes, alar base retraction, releasing the levator alaeque nasi muscle, the piriform ligament and the depressor septi nasi muscle, and maxillary hypoplasia.

• Chapter 6 – Revision Rhinoplasty: choosing the dissection plane after a previous operation, the closed approach for challenging cases, the saddle nose, rib and auricular cartilage grafts, costal reconstruction in cleft lip-nose deformity, the diced cartilage flap, restoring tip mobility, over-rotated noses and the fisherman suture.

• Chapter 7 – Advanced Rhinoplasty: the male and the female nose, an algorithm by skin type, thin- and thick-skinned patients, DUAL plane dissection, and Asian, African and Middle Eastern rhinoplasty.
