Turkey Neck: Why the Neck Sags and What Actually Changes It

6 min read

Quick answer: A sagging neck is produced by four different layers, not one. Loose skin, fat above the platysma muscle, the platysma muscle itself separating into visible bands, and deep fat or salivary glands below the muscle each cause a different shape of neck, and each responds to a different treatment. Creams, exercises and neck straps do not change any of them. Energy-based tightening can improve the skin surface a little. A deep plane neck lift repositions the muscle and treats the deep layer, which is what restores the angle between the chin and the neck.

Key takeaways

  • Turkey neck describes an appearance, not a cause; the plan depends on which layer is responsible.

  • Vertical cords in the neck are the free edges of a separated platysma muscle.

  • In most people over fifty, the fullness under the chin sits below the muscle, out of reach of liposuction.

  • The angle between chin and neck is restored by repositioning the deep layer, not by removing skin.

  • A sagging neck and hanging jowls usually appear together, which is why the two operations are often planned in one session.

What people mean by a turkey neck

Patients use the term for three different things: skin that hangs in a curtain under the chin, two vertical cords that stand out when speaking, and a neck that has lost the clear angle it used to have under the jaw. Often all three are present at once. Because the word describes the look and not the cause, two people with the same complaint can need two different operations.

The four layers that make a neck sag

  1. Skin. Sun damage, smoking and the loss of collagen over the decades reduce the skin's ability to recoil. Skin alone rarely produces the full picture, but it decides how smoothly the neck settles after surgery.

  2. Fat above the platysma. This is the subcutaneous layer that liposuction can remove. In younger patients with firm skin this is sometimes the whole problem.

  3. The platysma muscle. This thin sheet runs from the chest up over the jaw. With age its two halves separate in the midline and the free edges become visible as vertical bands. The muscle is not weak; it is separated and lengthened.

  4. Deep fat and the submandibular glands. Below the muscle there is a second fat compartment and, further back, the submandibular salivary glands. When these are full, the neck stays heavy even after everything above the muscle has been removed.

A surgeon works out which layers are involved by examining the neck while you clench the muscle, swallow and turn your head, not from a photograph.

Why creams, exercises and straps do not change the shape

Neck exercises are often recommended online. They train the platysma, but a platysma that has separated in the midline does not come back together by contracting it, and no exercise reaches the deep fat or the glands. Creams act on the surface of the skin and can improve texture and fine lines; they do not move tissue. Chin straps compress tissue for a few hours and the effect disappears when the strap is removed. None of this is harmful, and none of it alters the anatomy described above.

What non-surgical treatments can and cannot do

  • Botulinum toxin relaxes the platysma and can soften the pull of the bands in a neck that is otherwise in good condition. It does not remove skin or fat, and it is repeated every few months.

  • Energy-based skin tightening (radiofrequency, ultrasound, radiofrequency microneedling) works on the skin and the layer immediately beneath it. It can improve an early neck. It does not reposition muscle. In October 2026 the United States Food and Drug Administration issued a safety communication about serious injuries reported with radiofrequency microneedling devices, so these treatments deserve the same questions you would ask about surgery.

  • Deoxycholic acid injections dissolve a small amount of fat above the muscle. They do nothing for bands or for deep fat.

  • Fillers along the jaw can disguise an early jowl, but they add weight to a face that is already descending.

The honest summary is that non-surgical treatment suits an early neck and buys time. It does not reproduce what surgery does, and marketing that says otherwise is the main source of disappointment in this field.

What surgery actually does

A deep plane neck lift releases the deep layer, repositions the platysma and, where needed, treats the fat and the glands beneath it. The two halves of the muscle are brought together in the midline and secured, which recreates the angle under the chin. The skin is then laid back without tension rather than pulled. How this is planned is explained in deep plane neck lift and jawline definition.

Submental liposuction on its own is a reasonable operation for a younger patient with firm skin whose fat sits above the muscle only. Used on the wrong neck it empties the superficial layer and makes the bands and the deep fullness more visible.

Isolated platysmaplasty, closing the muscle through a small incision under the chin, suits a neck with bands but little loose skin.

Neck lift alone, or together with a facelift?

The neck and the jawline age together. When the jowls already hang over the jaw, lifting the neck on its own leaves a visible mismatch: a clean neck under an untreated face. When the jowls are still soft and the complaint is clearly the neck, a neck lift can stand alone. The comparison is set out in facelift versus neck lift.

Scars, recovery and time in Istanbul

The incisions run under the chin and around the ear, following the natural creases and continuing into the hair. Swelling peaks around the third day. Stitches are removed at about one week, and international patients stay in Istanbul for eight days until they are cleared to fly. Most people are back at office work at two to three weeks. Discomfort is usually described as tightness rather than sharp pain; this is covered in neck lift pain and recovery.

How to judge before and after photographs

  • Look at the side view with the chin in a neutral position, not lifted.

  • Check the angle under the chin, not only the skin.

  • Look for an earlobe that hangs freely, with no hair or beard pulled onto the ear.

  • Ask whether the photograph was taken at one week, three months or a year; swelling hides and flatters in different ways at each stage.

Who is a candidate

Good general health matters more than age. Blood pressure should be well controlled, smoking stopped before and after surgery, and weight stable. A neck that changes with large weight swings is better treated once the weight has settled. Candidacy, technique and the fee are decided only after an examination; you can read what a deep plane facelift involves if the face is part of the plan.

Frequently Asked Questions

What is a turkey neck?

It is the everyday name for a neck that hangs loosely under the chin, usually with vertical bands and a lost angle between the chin and the neck. It is not a medical diagnosis, and more than one layer is normally involved.

Can neck exercises get rid of a turkey neck?

No. Exercises work the platysma, but the muscle is not weak: it has separated in the midline and lengthened. Exercising it does not close that separation or remove the fat and loose skin above and below it.

Do neck creams or neck straps work?

Creams can improve the surface of the skin. Neither creams nor straps move the muscle, the deep fat or the salivary glands, so neither changes the shape of the neck.

Is a neck lift enough, or do I also need a facelift?

It depends on the jawline. If the jowls are soft and the neck is the main problem, a neck lift may be enough. If the jowls hang over the jaw, lifting the neck alone usually leaves the face looking untreated.

How long is the recovery after a neck lift?

Swelling peaks around the third day and settles over the following weeks. Stitches come out at about one week, and most people return to office work at two to three weeks.

Does liposuction alone fix a sagging neck?

Only in a small group of younger patients with good skin and with fat sitting above the muscle. In most people over fifty the fullness lies below the platysma, where liposuction cannot reach.
