Lymphatic Drainage After Facelift: When to Start, What to Avoid
Updated: a few seconds ago
Quick answer. Gentle manual lymphatic drainage is widely used after facelift surgery to help swelling settle, but there are no randomised trials in facelift patients. A facelift temporarily interrupts the face's lymph channels; one imaging study found they had largely recovered by three months and fully by six. Start only when your surgeon agrees, with a trained therapist, using very light pressure and never deep massage.
Key takeaways
Swelling after a facelift is partly due to temporary disruption of lymph channels.
Direct evidence for lymphatic drainage after facelift is limited; most data come from other facial surgery.
In those studies, measured swelling settled faster, but patients did not always feel a difference.
Pressure should be feather-light and directed by a therapist trained in post-surgical care.
Deep massage, facial tools and heat devices on the healing face should be avoided unless your surgeon approves.
Why does a facelift cause swelling?
Lymph is the clear fluid that drains from tissues back into the circulation through a fine network of channels. When the skin and deeper layers of the face are lifted, many of these channels are interrupted.
A 2012 study in the Aesthetic Surgery Journal traced lymph flow with a mild radioactive tracer before and after facelift surgery in three women, each having a different technique (Meade et al., 2012). All three techniques temporarily disrupted lymphatic drainage to a similar degree. Drainage had largely recovered within three months and returned to its pre-surgery pattern after six months. The authors concluded that the extent of dissection, rather than its depth, mattered most for swelling. Three patients is a very small study, so treat these timings as a guide, not a rule.
An earlier surgeon's report found no lymphatic absorption on imaging in one patient one week after an extended facelift (Mottura, 2002). The same report found that a tight elastic bandage around the neck raised venous pressure in the face.

What does the evidence say about lymphatic drainage?
There are no randomised controlled trials of manual lymphatic drainage after facelift. The closest data come from other facial operations.
In a double-blind trial of 30 patients after jaw (orthognathic) surgery, drainage started on day 2 (Yaedú et al., 2017). The treated group's measured swelling went down faster and further than the placebo group's. However, there was no difference in pain or in how swollen patients felt.
In a 2025 split-mouth trial of 46 patients after wisdom tooth removal, swelling, pain and jaw stiffness were lower on the side that received drainage (Ulu et al., 2025).
A 2025 consensus panel of seven plastic surgeons and one dermatologist recommended lymphatic massage after facial surgery for patients with a history of non-surgical treatments, prolonged swelling or compromised blood supply (Shridharani et al., 2025). That is the panel's opinion, based mainly on data from body procedures, rather than facelift trial evidence.
Taken together, gentle drainage looks low-risk and may speed up measurable swelling reduction. It is not proven to change your final facelift result.
When can you start lymphatic drainage after a facelift?
Only when your surgeon clears it. Timing depends on your technique, your drains, your swelling and how your incisions are healing.
In one published protocol, manual lymphatic drainage began on the third to fifth day after facelift, done by a trained professional (Mottura, 2002). The author stressed that the method after facelift differs from the method taught for people who have not had surgery, because the normal drainage routes have been changed.
First days — What usually happens: Swelling builds; drains and dressings may be in place — Lymphatic drainage: Only if your surgeon specifically starts it
Late first week to second week — What usually happens: Swelling begins to settle; stitches being removed — Lymphatic drainage: Possible, if cleared and done by a trained therapist
Weeks to months — What usually happens: Residual swelling, often uneven — Lymphatic drainage: Some patients continue sessions or learn gentle self-drainage

How should it feel?
Lymphatic drainage is not a massage in the usual sense. The therapist uses very light, slow, skin-stretching movements, often starting at the neck to open drainage areas before working on the face. It should not hurt and should not leave marks.
The therapist should know which areas were lifted, where your incisions are and which direction your surgeon wants fluid moved.
What should you avoid after a facelift?
Unless your surgeon tells you otherwise, avoid:
deep tissue or sports massage on the face and neck
pulling or stretching across incisions
gua sha stones, rollers and other tools pressed firmly into the skin
heat, radiofrequency or other energy devices on the healing face
spa facials by therapists without post-surgical training
tight elastic garments or wraps around the neck beyond what your surgeon prescribes
Lymphatic drainage relies on light pressure; firm pressure is not part of the technique. Numb skin also cannot warn you about pressure or heat, so you may not notice harm as it happens.

When is swelling not normal?
Most facelift swelling is soft, roughly even on both sides and improves week by week. MedlinePlus notes that mild swelling may continue for several weeks (MedlinePlus, n.d.). Contact your surgeon, and do not book a drainage session, if you notice:
one side swelling much more than the other, quickly
firm, tight or increasingly painful swelling
redness, warmth, discharge or fever
new bruising that spreads after the first few days
These can signal a blood collection (hematoma) or infection, which massage will not fix. If you have difficulty breathing or swallowing, call emergency services.

Frequently Asked Questions
Does lymphatic drainage reduce facelift swelling?
It may help swelling settle faster. Trials after jaw and wisdom tooth surgery found measurably less swelling with drainage, but one found no difference in how swollen patients felt. There are no randomised trials in facelift patients yet. Think of it as a gentle support, not a treatment that changes your final result.
How many lymphatic drainage sessions do I need after a facelift?
There is no evidence-based number. Some patients have a few sessions in the first weeks; others have none and recover well. Your surgeon and therapist decide based on how much swelling you have and how it changes. Stop and contact your surgeon if sessions are painful or swelling increases.
Can I do lymphatic drainage myself at home?
Possibly, once your surgeon allows it and a trained therapist has shown you the correct direction and pressure. Use clean hands, feather-light strokes and no tools. Never press over incisions or areas that are numb, hard or very tender. If you are unsure, do less, not more.
Is lymphatic drainage safe in the first days after a facelift?
Very early drainage is done only if your surgeon specifically prescribes it. In the first days, tissues are fragile, drains may be in place and hematoma is the main risk to watch for. One published facelift protocol started drainage on day 3 to 5, using a technique adapted for operated patients.
How long does facelift swelling last?
Visible swelling settles over several weeks, but subtle swelling can last for months. An imaging study found lymph drainage largely recovered by three months and fully returned to normal by six months after facelift. Uneven swelling during this time is common and usually resolves without treatment.
