Case Study 17: Male Rhinoplasty with Fractional CO2 Laser for a Humped, Wide Nose

The patient's concerns

This patient had a dorsal hump, a wide nose and breathing problems. As a young man, he wanted the change to look natural and masculine, not operated.

About the surgeon

Prof. Dr. Suleyman Tas is a board-certified plastic, reconstructive and aesthetic surgeon with more than 20 years of experience and thousands of rhinoplasties, including closed, open, preservation and revision techniques. He is the author of the textbook Rhinoplasty in Practice and trains surgeons internationally.

What was done

Rhinoplasty

The hump was removed and the dorsum was shaped into a straight, strong male bridge. The nasal bones were narrowed to reduce the width while keeping enough size to preserve masculinity, and the internal structures responsible for his breathing problem were corrected in the same operation. Male rhinoplasty requires particular care with the tip angle, which should stay close to 90 degrees, and with avoiding over-reduction. Read more in male rhinoplasty vs. female rhinoplasty.

Fractional CO2 laser

Men often have thick, oily skin with enlarged pores, which can hide the refinement achieved by rhinoplasty. Fractional CO2 laser treats the skin surface and stimulates collagen in the deeper connective tissue, improving texture and helping the skin tighten and recover. The same laser can also be used later to refine scars or skin irregularities if needed.

Why combine rhinoplasty and CO2 laser?

Combining surgery with laser treats both the framework and the envelope of the nose: the structure is reshaped, and the skin is encouraged to adapt to it with better quality and tone.

Result

The patient achieved a straight, narrower, masculine nose, easier breathing and smoother skin. See more in the male rhinoplasty before and after gallery.

Frequently asked questions

Does CO2 laser increase recovery time after rhinoplasty?

Only slightly; skin redness typically settles within one to two weeks.

Can thick male skin be refined?

Skin quality can be improved and swelling managed, but thick skin always needs longer to show definition.

Will my nose look feminine after hump removal?

Not when the bridge is kept straight, the tip is not over-rotated and the size stays appropriate.

How soon can I go back to work after nose surgery?

Most patients return to desk work once the cast is removed at about a week, when the swelling and bruising around the eyes have largely faded.

Will my result look like this one?

Every nose starts from different anatomy — skin thickness, cartilage strength and any previous surgery change what is achievable. A case shows what was possible for that patient, not a promise for every nose.

When can I exercise after nose surgery?

Light walking in the first week, gentle cardio after three to four weeks, and contact sports or anything that risks a blow to the nose only after about three months.

Does the result change over the years?

The nose ages with the face: skin thins and the tip settles slightly over decades. A nose whose support has been preserved or rebuilt keeps both its shape and its airway.
