Case Study 4: Male Revision Rhinoplasty with Mid-Face, Temporal and Neck Lift

The patient's concerns

This patient had undergone rhinoplasty elsewhere and was unhappy with the result. His tip was too pointed, the base of the nose was wide and the columella hung below the nostrils, a combination that looked operated and unmasculine. At the same time he was bothered by a double chin and sagging skin on the face and neck that made him look older and heavier than he felt.

The plan

Men are often more concerned than women that surgery will be noticeable. The plan therefore focused on restoring masculine proportions to the nose and on structural lifting of the face rather than tightening the skin, so that the result would read as healthy and rested, not "done".

What was done

Revision rhinoplasty

The overly pointed tip was rebuilt into a stronger, defined but natural male tip; the wide base was narrowed; and the hanging columella was corrected so the nostrils and columella sit in harmony in profile. In revision cases, scar tissue and previously removed cartilage often require grafting to create lasting support. See more male revision rhinoplasty results.

Mid-facelift and temporal lift

A mid-facelift (cheek lift) raised the descended cheek tissue, restoring the contour of the cheekbones and softening the lines between the nose and mouth. A temporal lift elevated the outer brow and temple area to refresh the eyes without feminising the brow shape.

Neck lift

The neck lift removed excess skin and fat under the chin, tightened the underlying muscle and recontoured the jaw-neck angle, eliminating the double chin and restoring a defined male jawline. Learn more on the facelift page and the jawline page.

PAX laser and nano-fat injections

PAX laser treatment improved skin texture and tone, while nano-fat, a highly purified form of the patient's own fat, was injected into selected areas to restore volume and improve skin quality.

Result

The patient left with a natural, masculine nose that no longer looks operated, a tighter and more defined jawline and neck, and healthier skin. The combination of structural revision and facial lifting produced a coherent, rejuvenated appearance.

Frequently asked questions

Can a pointed tip from a previous rhinoplasty be corrected?

Yes. The tip is reconstructed with cartilage support to give it a stronger, more natural shape.

Does a neck lift leave visible scars?

Incisions are placed behind the ears and in the natural crease under the chin, where they are well hidden.

How long do men need to recover from combined surgery?

Most men return to work within two to three weeks; swelling in the nose continues to settle over the following year.

How long do facial rejuvenation results last?

A surgical lift typically holds for around a decade because it is carried by the deeper layer rather than the skin. Ageing continues, so sun protection, skin care and non-surgical maintenance still matter.

Where are the scars?

In the natural creases in front of and behind the ear and inside the hairline, placed so that hair and make-up cover them while they fade over the following months.
