Methods and Clinical Approach

Seven tertiary rhinoplasty patients with a dorsal gap smaller than 1 cm were selected from 12 referred cases. The adherent diced-cartilage island was released distally, preserved on its proximal soft-tissue attachment, advanced caudally and secured behind the dome region.

Results

Follow-up ranged from 12 to 20 months. The surgeon and patients reported satisfactory contour, tip projection and rotation; no graft resorption was observed.

Clinical Significance

The diced cartilage flap offers a practical autologous option for selected lower-third saddle nose defects after prior diced-cartilage graft failure. Evidence level IV.
